Indications/Uses
Depression.
Dosage/Direction for Use
Adult Initially 15 mg/day, may gradually increase based on response. Usual range: 15-45 mg/day as 1-2 divided doses.
Administration
May be taken with or without food: Place tab on tongue & allow to disintegrate then swallow w/ water or other liqd.
Special Precautions
Epilepsy or history of seizures; avoid completely in unstable cases. Cardiac disorders eg, conduction disturbances, angina pectoris, recent MI. Hypotension, DM. History of bipolar disorder, psychoses. Discontinue treatment if jaundice develops. Micturition disturbances, angle-closure glaucoma, raised IOP. Monitor for signs of bone marrow depression & suicidal tendency. Avoid abrupt w/drawal. May impair ability to drive or operate machinery. Hepatic or renal impairment. Pregnancy & lactation. Elderly.
Adverse Reactions
Increased appetite & wt, oedema; drowsiness or sedation; dizziness, headache, increased liver enzyme levels; jaundice. Orthostatic hypotension, rash, nightmares, agitation, mania, hallucinations, paraesthesia, convulsions, tremor, myoclonus, akathisia, restless leg syndrome, arthralgia, myalgia, reversible agranulocytosis, leucopenia, granulocytopenia, hyponatremia.
Drug Interactions
Potentiated sedative effects w/ alcohol or benzodiazepines. Increased plasma levels w/ potent CYP3A4 inhibitors (eg, HIV-PIs, azole antifungals including ketoconazole, erythromycin, nefazodone). Reduced plasma levels w/ carbamazepine & other inducers of CYP3A4. Increased bioavailability w/ cimetidine. May provoke a serious reaction w/ MAOIs eg, phenelzine.
